A
medical home for children with special needs is an approach
to providing healthcare services; it is not just a building
or office. The principles that guide the medical
home model include accessibility, family-centeredness,
continuous, comprehensive, coordinated, compassionate and
culturally competent care.
An
organization serving Southern Arizona including Pima, Cochise,
Santa Cruz, Graham, Greenlee counties as well as parts of
Pinal and Gila counties. Children's Clinics also serves patients
from other parts of Arizona when they have been referred for
specialty or tertiary services. Patients from neighboring
states and Mexico may be seen when special payment arrangements
have been made or when care is covered by insurance.
The
multidisciplinary approach to care implemented at Children's
Clinics allows for collaboration and coordination of care
among professionals while, at the same time, eliminating the
need for multiple doctor visits for the families. There is
a partnership among doctors, nurses, therapists, and social
service professionals. Child life services, educational advocacy
and nutritional counseling are also available on site. This
approach enables the development and implementation of a comprehensive
care and management plan, which leads to quality outcomes
for the child. This is especially true of the multispecialty
clinic teams.
Located
in Tucson, Arizona
The
Southern Arizona contractor for the Arizona
Department of Health Services (ADHS), Children's
Rehabilitative Services (CRS) program of the Office
of Children with Special Health Care Needs (OCSHCN). The
CRS program uses a combination of State of Arizona and Federal
health care dollars to provide services to children with special
health care needs. Based upon the medical condition and financial
status of the child, he or she may be eligible to receive
services funded under the CRS program. Other CRS contractors
include St. Joseph's
Hospital and Medical Center in Phoenix, serving Central
Arizona; Yuma Regional
Medical Center, serving Western Arizona; and Flagstaff
Medical Center, serving Northern Arizona.
CRS
has produced a handbook that has helpful information about
the CRS Program. Members have a right to receive the CRS Handbook.
This is given out to new members at the intake appointment.

Children
from birth to age 21 can receive services at Children's Clinics.
Upon turning 21, patients are assisted in transitioning to
other agencies and health care plans for their continuing
care, as indicated by their medical condition.
The
Parent Action Counsel (PAC) is open to parents, foster parents,
family members and legal guardians of children who have a
relationship with the Children's Clinics. The goals of PAC
are to advise, support, advocate for, educate and unite those
who care for special needs children. PAC promotes parent involvement
in treatment planning, advocacy and patient care. The Parent
Action Council is an avenue through which parents can share
concerns and where ideas and information are exchanged. Through
regular meetings PAC provides a network and format for encouraging
improvement in the care and treatment of children with special
health care needs. When needed, PAC works to change laws,
rules, policies and procedures that directly influence the
delivery of services to families, including ongoing definitions
of parent/patient rights and responsibilities. The local PAC
provides input to the State Parent Action Council (SPAC) for
the Department of Health
Services for the planning and delivery of services through
the Children's
Rehabilitation Service (CRS) program.
Ability
to be seen at Children's Clinics is based on the existence
of a medically qualifying condition that is appropriate for
the types of services that are offered here. The process is
described on the Enrollment
page.
Private
insurance may cover services at Children's Clinics, but plans
differ with respect to which physicians are on their panels
and what facilities they use for inpatient and laboratory

will be reviewed thoroughly at the payment
responsibility interview as well.
Children’s
Clinics is accredited by the Joint Commission and maintains
high standards of patient care and safety. We are concerned

Continuous
enrollment refers to the process of re-enrollment that is
necessary to maintain ongoing care at Children's Clinics.
Monthly levels of patient enrollment determine state funding
for programs such as AHCCCS,
Kidscare and
CRS.
Missing re-enrollment, even by as little as one month, affects
the financial support of the organization and in turn, the
ability to provide continuous services.
It is
easy to keep enrollment current. Re-enrollment can be processed
- at
local AHCCCS offices OR
- by
sending KidsCare renewal paperwork in on time OR
- by
coming to the Enrollment/DES office at Children's Clinics.
It makes
a difference for all of the children receiving services at
Children's Clinics.
